Understanding Your Options
The right next step depends on the individual — their independence, health, safety, preferences, finances, family support and goals.
Sometimes moving isn't the answer.
Many families find that the right next step is additional support at home. Families may explore:
For older adults who are generally able to live independently but may want community, social connection, activities, convenience and fewer home-maintenance responsibilities.
Independent living communities are generally not licensed Oregon long-term-care settings and are a different category from assisted living, residential care, adult foster homes or nursing facilities.
Generally, assisted living combines housing with supportive services and assistance with certain activities of daily living, along with staffing available on site.

Memory-care settings may provide additional structure, supervision and services for people experiencing cognitive or memory-related challenges.
We do not diagnose dementia or Alzheimer's disease and we never tell a family they need memory care. If memory changes are part of your situation, memory-care options may be worth exploring alongside a conversation with a qualified healthcare professional.
Adult foster homes can offer care in a smaller, residential environment, often with a small number of residents in a home-like setting.
Generally, Oregon adult foster homes serving older adults and adults with physical disabilities are licensed residential settings.
Residential-care facilities are another type of licensed residential setting that may provide housing along with supportive care services.
Nursing facilities can provide higher levels of nursing care and rehabilitative services, often following a hospital stay or a change in medical needs.
We do not make medical eligibility determinations. Clinical decisions are made by appropriate healthcare professionals.
Continuing-care communities may offer multiple levels of living and care within one community or campus, which some families consider when planning ahead.
Families facing serious illness or end-of-life decisions may need help understanding hospice, palliative-care and supportive resources. These conversations deserve patience, and no family should have to sort through them alone.
Legacy Navigation does not diagnose medical conditions or determine hospice eligibility. Hospice eligibility and clinical decisions must be made by appropriate healthcare professionals and hospice providers.

Description of our referral services
Legacy Navigation is a licensed long-term care referral agency with the State of Oregon, Oregon Department of Human Services, serving Southern Oregon. At your request, we may refer you to licensed long-term care settings in Oregon, such as assisted living, residential care, memory care and adult foster homes, and help you compare options. There is never a direct charge to your family for our referral services. Legacy Navigation may receive compensation from participating providers when an individual chooses and moves into a participating care setting. Information regarding our services, provider relationships and compensation is provided before a long-term care referral is made. Referral decisions are always made by the individual and family; Legacy Navigation does not guarantee availability, acceptance, placement, quality or medical appropriateness at any setting.
Compensation disclosure
There may be no direct charge to the family for certain referral services. Legacy Navigation may receive compensation from participating providers when an individual chooses and moves into a participating care setting. Information regarding our services, provider relationships and compensation will be provided before a long-term care referral is made.
Legacy Navigation may refer to facilities and providers with which it has contractual relationships, and will disclose those relationships before a referral is made.
