Privacy Policy

Legacy Navigation is a senior navigation and resource network serving families in Southern Oregon. This Policy explains how we handle the personal, care, and financial details you share with us and how you stay in control of where your information goes.

1. About Legacy Navigation

Legacy Navigation (“Legacy Navigation,” “we,” “us,” or “our”) is an independently operated Oregon business and limited liability company providing senior navigation, concierge resource referrals, and long-term-care referral and placement services to older adults and families in Southern Oregon.

We help individuals and families understand their options, identify resources, and navigate important life transitions involving senior living, caregiving, Medicare, estate planning, and other trusted community resources.

Legacy Navigation may collaborate with Bulletproof Legacy Financial Services and other independent professionals when a client specifically requests those services. Those businesses are separate entities, and we share information with them only when you ask us to or otherwise authorize the sharing.

Legacy Navigation is a licensed long-term care referral agency with the State of Oregon, Oregon Department of Human Services (Certificate #2444, effective September 16, 2026).

This Privacy Policy explains what information we collect, how we use it, when and how we share it with providers and referral partners, and the choices and rights you have. It applies to this website, our phone and email communications, our consultations, and our events.

Oregon Licensing Status

Legacy Navigation is a licensed long-term care referral agency with the State of Oregon, Oregon Department of Human Services. Certificate #2444, effective September 16, 2026 and current through September 16, 2028.

2. Why We Ask, and What We Collect

Before we ask for personal information, we explain why we need it. We collect information for one purpose: to understand your situation, identify suitable options and resources, and — only with your permission — make introductions you have asked us to make. If you are ever unsure why we are asking for something, ask us and we will tell you, and you may decline.

We collect information lawfully and fairly, directly from you or from someone you authorize, always with your knowledge and consent. We do not gather personal information about you from facilities, data brokers, or other sources behind your back.

We collect only what we need to guide your family and identify appropriate resources:

  • Contact information — name, phone number, email address, mailing or property address, and your relationship to the older adult (self, spouse, adult child, power of attorney, guardian, professional).
  • Care and situation details you choose to share — current living arrangement, mobility, activities of daily living, memory or cognitive needs, behavioral considerations, diagnoses or medications you voluntarily share, preferred location, timeline and urgency, and other health-related information relevant to identifying resources.
  • Financial-suitability information you choose to share — approximate monthly budget and funding sources such as private pay, long-term care insurance, VA benefits, Medicaid or Oregon Medicaid (including K Plan) eligibility status, and home-sale considerations. We do not request Social Security numbers, bank account numbers, or full insurance policy numbers through this website.
  • Consultation and navigation records — notes from calls and meetings, the roadmap or option list we prepare, which providers you asked us to contact, tour or appointment scheduling details, and the outcome of an introduction.
  • Partner and professional information — for network applicants, business and licensing details, insurance and background-screening information, and the individuals authorized to receive introductions.
  • Website and scheduling data — form submissions, appointment requests, and basic analytics such as pages viewed, approximate region, browser type, and referring link.

You are never required to give us health information to use this website. Please share only what you are comfortable sharing, and only about a person you are authorized to represent.

3. The Care Compass™

Legacy Navigation may use The Care Compass™ and related consultation forms to better understand an individual’s circumstances, needs, preferences, priorities, desired location, timeline, lifestyle considerations, care or support needs, and financial considerations relevant to identifying potential resources.

Information collected through The Care Compass™ is used to provide personalized navigation and resource assistance.

Completing The Care Compass™ does not constitute blanket authorization for Legacy Navigation to share placement information with senior living communities, care providers, or other businesses.

When authorization is required before information is shared with a particular facility or recipient, Legacy Navigation will obtain that authorization separately.

Legacy Navigation does not use The Care Compass™ to perform medical or clinical assessments or determine an individual’s required level of care. Senior living and care providers remain responsible for conducting their own assessments and determining whether they can appropriately serve an individual.

4. How We Use Your Information

  • To respond to your inquiry and understand your situation.
  • To identify suitable options and prepare your personalized option list or roadmap.
  • To make introductions you have authorized to providers and network partners, and to follow up on how those introductions went.
  • To coordinate tours, appointments, and hand-offs between you and a provider.
  • To communicate with you about your active request, and — if you opt in — about resources, events, and seminars. You may unsubscribe at any time.
  • To maintain our records, improve our services and partner standards, and comply with legal, registration, insurance, and audit obligations.

We do not use your information for third-party advertising.

We do not sell or rent your personal information to anyone. Compensation we may receive from a provider after a client chooses that provider is a referral fee for a service, not a sale of your personal information.

5. Referral Relationships and Compensation

Our guidance and resource referrals are provided at no cost to families.

Legacy Navigation is a licensed long-term care referral agency with the State of Oregon, Oregon Department of Human Services (Certificate #2444).

For certain referrals to licensed long-term care settings, Legacy Navigation may receive a referral or placement fee from the facility if the person becomes a resident or client. Families are not charged a fee by Legacy Navigation for those referrals.

Legacy Navigation may also have separate sponsorship, advertising, directory, membership, or partnership arrangements with businesses in its broader resource network. Those arrangements are distinct from client-specific long-term care referrals and do not authorize the sharing of a client's personal information.

Compensation relationships do not guarantee placement, endorsement, or suitability. Families remain free to choose any provider or facility, and a client’s freedom to consider, contact, tour, or select a senior living community is not conditioned upon whether that community has a financial relationship with Legacy Navigation. If you would like to know whether a specific provider has a compensation relationship with us, ask and we will tell you in writing.

Legacy Navigation provides long-term care referral services in accordance with its Oregon license and applicable Oregon law.

Any required disclosures regarding referral relationships and compensation will be provided to clients in accordance with applicable Oregon law.

6. Sharing Information With Senior Living Communities and Referral Partners

Legacy Navigation believes individuals and families should remain in control of where their placement information is sent.

Before we begin referral services, we give you a written Disclosure and Advisory Form together with this privacy statement, review it with you, and ask you to sign it. That form explains our services, our compensation relationships, and your choices, and it records the permission you give us.

Your personal information is shared only with the facilities or providers you are actually being referred to — the ones you have named or agreed to. It is not circulated to our wider network, our advertisers, or any other business.

Legacy Navigation will not provide a client’s personally identifiable information, health or care information, financial-suitability information, or contact information to a long-term-care facility or referral partner without authorization from the client or the client’s legally authorized representative. The client may limit the information shared.

Before sharing a client’s placement information with a long-term care facility, home, or applicable marketing affiliate, Legacy Navigation obtains the client’s or authorized representative’s clear, affirmative consent for each instance of sharing.

Authorization may identify the specific facility, community, provider, or other authorized recipient with whom the client has agreed information may be shared.

Legacy Navigation will share only information reasonably relevant to the requested introduction or referral and consistent with the authorization provided by the client or authorized representative.

A general inquiry, Care Compass™ submission, consultation request, or consent to communicate with Legacy Navigation does not automatically authorize Legacy Navigation to distribute a client’s placement information throughout its provider network.

Clients remain in control of which facilities or providers they would like Legacy Navigation to contact on their behalf.

Apart from authorized introductions, we share your information only in these circumstances:

  • With people you designate, such as a spouse, adult child, agent under a power of attorney, guardian, conservator, or professional advisor you tell us to include.
  • With service providers who operate our business under confidentiality obligations — for example, our website host, scheduling and calendar tools, email provider, and CRM. They may process your information only on our instructions.
  • With affiliated or collaborating businesses, including Bulletproof Legacy Financial Services, only when you specifically request those services or otherwise authorize the sharing.
  • When required or permitted by law — including responding to lawful requests from courts, regulators, or the Oregon Department of Human Services, and cooperating with mandatory reporting of suspected abuse, neglect, or exploitation of a vulnerable adult under Oregon law (ORS 124.050–124.095).
  • To protect safety, prevent fraud, or enforce our terms; and in connection with a sale or reorganization of our business, subject to this Policy.

We do not share your details with a provider who you have not asked us to contact.

Oregon Licensing Status

Legacy Navigation is a licensed long-term care referral agency with the State of Oregon, Oregon Department of Human Services (Certificate #2444). Client information is shared with a facility only with the client’s authorization.

7. The Legacy Navigation Partner Network

Legacy Navigation operates or may operate the Legacy Navigation Partner Network, a network of businesses, professionals, nonprofit organizations, community resources, senior-serving organizations, and other providers.

Participation in the Legacy Navigation Partner Network, directory, partnership program, sponsorship program, advertising program, event, or other marketing relationship does not authorize a participating organization to receive a Legacy Navigation client’s placement information or other nonpublic personal information.

Partnership, sponsorship, advertising, or other participation in the Legacy Navigation Partner Network is separate from any authorization a client may provide for Legacy Navigation to share information with a particular facility or provider.

Legacy Navigation shares client placement information only in accordance with this Privacy Policy, applicable law, and any required client or authorized-representative consent.

8. Care and Health Information

Legacy Navigation is a navigation and resource service, not a health care provider, health plan, or health care clearinghouse. Even so, we treat health and care details you share as sensitive and confidential: access is limited to staff who need it, we ask for written authorization before disclosing those details to a provider, and we do not disclose health details for marketing.

We use reasonable administrative, technical, and physical safeguards appropriate to the information we hold.

If you ask us to obtain records or care assessments from a provider on your behalf, we will ask you (or your authorized representative) to sign a separate written authorization naming the provider and the information involved. You may revoke that authorization at any time in writing, except as to disclosures already made.

9. Authority to Share Another Person's Information

If you contact us about a parent, spouse, or client, you confirm that you are that person’s legal representative or that they have consented to your sharing their information with us. If we learn that an older adult has not consented and has capacity to decide, we will ask for their permission before proceeding, and we may limit what we discuss.

10. How Long We Keep Information

We keep each client’s signed Disclosure and Advisory Form for three years.

We do not keep sensitive details — such as health, care, or financial-resource information — any longer than we need them. Once your search is complete or you tell us you are finished, we securely destroy or de-identify those details rather than holding them indefinitely.

We retain basic consultation notes, navigation records, and correspondence for as long as needed to serve your family and to satisfy legal, tax, insurance, and audit requirements — generally up to seven years after your last interaction with us — and then securely delete or de-identify them. Marketing contact information is kept until you unsubscribe. Partner records are kept for the duration of the partnership plus applicable retention periods.

11. Keeping Your Information Accurate

The information we hold about you should be accurate, complete, and current, because we rely on it to identify the right options for your family. Please tell us right away if anything changes or looks wrong — a phone number, a budget range, a care need, or who is authorized to speak for you — and we will update our records promptly and, where appropriate, let a provider you authorized know about the correction.

12. How We Protect Information

Legacy Navigation takes reasonable administrative, technical, physical, and organizational measures to protect personal information against loss, theft, and unauthorized access, use, disclosure, copying, or modification. These include encrypted transmission of website form data, password-protected and access-controlled systems with least-privilege permissions, confidentiality agreements with staff and partners, physical files kept in locked storage, secure destruction of records we no longer need, and review of the vendors who handle information for us.

Access to nonpublic client information is limited to individuals who reasonably need access to perform authorized Legacy Navigation functions.

No method of transmission or storage can be guaranteed against every risk, so please do not send Social Security numbers, account numbers, or detailed medical records by unencrypted email.

If a breach of Oregon residents’ personal information occurs, we will notify affected individuals and the Oregon Department of Justice as required by the Oregon Consumer Identity Theft Protection Act (ORS 646A.600–646A.628).

13. Your Choices and Control

You may contact Legacy Navigation to:

  • Ask what personal information we maintain about you;
  • Request correction of inaccurate information;
  • Ask questions about how your information has been used or shared;
  • Withdraw authorization for future sharing of information, subject to information already disclosed pursuant to a valid authorization;
  • Request that Legacy Navigation stop contacting you; or
  • Request other privacy-related assistance.

Requests can be made by phone or email using the contact details at the end of this Policy; Rachel Kalb handles privacy requests personally. We acknowledge requests promptly, make corrections or updates as soon as we verify them, and tell you what we did.

If you notify Legacy Navigation in writing that you no longer wish to receive navigation or referral services, Legacy Navigation will honor that request in accordance with applicable law.

14. Oregon Consumer Privacy Rights

Oregon residents have rights under the Oregon Consumer Privacy Act (ORS 646A.570–646A.589), and we honor the following requests from any family we serve:

  • Confirm whether we process your personal data and obtain a copy of it.
  • Correct inaccurate personal data.
  • Delete personal data we hold about you, subject to our legal retention obligations.
  • Obtain a list of the specific third parties, or categories of third parties, to which we have disclosed your personal data.
  • Opt out of targeted advertising, the sale of personal data, and profiling for decisions with significant effects. We do not engage in any of these activities.
  • Withdraw consent to share your information with a provider or referral partner, and unsubscribe from our communications.
  • Appeal a denied request. If we decline, we will explain why, and you may appeal by replying to our decision; you may also contact the Oregon Department of Justice.

To exercise a right, contact us using the details below. We will verify your identity or authority — including authority to act as an authorized agent or on behalf of an older adult — and respond within 45 days, with one 45-day extension if needed. Exercising these rights will never affect the quality of the guidance you receive from us.

15. Children's Information

Our services are directed to adults. We do not knowingly collect information from anyone under 13, and we do not process the personal data of anyone we know to be under 16 for targeted advertising or sale.

16. Cookies, Analytics, and Do Not Track

This site uses essential cookies for basic functionality and limited analytics to understand which resources families find helpful. We do not use advertising cookies or cross-site tracking pixels, and we do not sell website data. You can block or delete cookies in your browser settings. Because we do not conduct targeted advertising, we treat Global Privacy Control and Do Not Track signals as consistent with our existing practices.

17. Third-Party Sites

Our directory and resources link to partner and government websites, including licensing lookups. Those sites have their own privacy practices, and this Policy does not apply to them.

18. Changes to This Policy

We may update this Policy to reflect changes in our services or the law, including when our Oregon licensing status changes. We will post the revised version here with a new effective date, and we will notify you directly if the changes materially affect how we share information about you.
